Living Horizon has a strong reputation throughout the UK, as a leading supported living provider, working with residents with forensic and the most challenging of backgrounds.
Established in 2012 - from the outset we have been supporting those with complex mental health needs. We have found our service users have had some of the longest placements in their adult lives with us. Find out more about our teams and how we support our residents for success.

Who We Support
As a 24/7 supported living care provider for complex and forensic mental health needs, we support individuals from all walks of life - including those coming from secure hospitals, having completed custodial sentences or homelessness.
Challenging Behaviours
Our team is experienced in providing boundaried and compassionate person-centred care that focuses on independent living. Through our positive behaviour support plans, over time, we help reassure, support and stabilise the most challenging of residents.
Emergency Placements
We are often the last line of defence for commissioners seeking a specialist provider, before options of return to a secure setting or homelessness. We're always ready to discuss solutions.
Complex Diagnoses
We have experience supporting those with personality disorders, paranoid schizophrenia, bi-polar, all forms of psychosis, acquired brain injuries, autism, learning disabilities, dual diagnosis, self harm, drug/alcohol abuse, PTSD, eating disorders and more...

Referral & Assessment

Challenging Placements – we can assess in-person within days

Comprehensive Assessment Report – to quickly determine specific care needs and placement feasibility

Emergency placements – we try to keep beds available for exactly these scenarios
Stability During Transitions

We aim to provide stability as soon as possible, as very often our service users come from chaotic backgrounds

The move period often causes upset for complex needs service users, and our team work with them to make this as easy as possible for them to settle in, establish trust and routines, and find stability
Person Centred Care Plans

Based on the often complex needs of individuals, goals of care coordinators, local authorities and next of kin – we build a person centred care plan with service users

This is reviewed regularly and updated as service users settle in and make progress
Long Term Tenancies

We have regular care plan reviews with residents, and regular professionals meetings – to monitor and ensure that goals are being achieved and progress made for long term successful placements

Our ultimate goal is for service users to be calm, independent, and supported in their life goals; whether this means they move on to live independently or retain a long term tenancy
